Clinical services between appointments
(communication with patient or family, letter or form completion, etc.) are billed at the rate of $50 per 10 minute increment. There is no charge for refill requests.

Appointments require 48-hours business days notice to reschedule or cancel, or $150 fee will be applied.

New Patients

You must be located in North Carolina or Connecticut where Dr. Matlock is licensed.

Please note this practice is fee-for-service and is out-of-network with insurance companies. This practice does not accept insurance or negotiate with insurance companies. A superbill will be provided that you may submit to insurance for partial reimbursement, though reimbursement cannot be guaranteed.
